• You must be 65 years or older; however some under the age of 65 may qualify due to a disability.
• Your physician must determine you need home care and construct a plan of care for you individually.
• You need at least one of the following medical assistance on a partial or full time basis; Nursing care, Physical Therapy, Occupational Therapy, Speech Language Pathology, Medical Social Worker, Home Health Aide, or Homemaker/attendant care.
• You must be incapable to leave home without considerable taxing effort.
• Physician must frequently review your individual plan on care.

• Address your doctor, social worker, or hospital discharge planner about home care agencies in your area.
• Get in touch with your state's department of health or social services to attain a list of licensed agencies in your area.

Who wouldn't want to stay home if they could? Home care is better because:

• it is more affordable than nursing homes or other institutions (most services are covered 100% by Medicare)
• it provides for faster release from the hospital
• it allows for patients and family members to be more involved with the patient's treatment
• being at home is more comfortable and helps with patient morale
• family and friends are close by and can visit anytime
• research shows that recovery can be faster at home than in the hospital.
