In addition to traditional home care, Diamond Home Care specializes in treating individuals who are recovering from accidents or injury such as:

 Diamond's holistic approach to care and experience in management of straight forward to complex medical cases makes them an excellent choice. Our team focuses on the individual's recovery, family education and support. We maintain consistent communication with necessary agencies and physicians through written progress reports and care conferences to facilitate coordination of care.

THE SKILLED TEAM AT DIAMOND HOME CARE OFFERS:

♦ NURSING - A team of highly skilled RNs who work closely with physicians, case managers and adjusters to assist in the coordination of care and medical management of the individual. Our nurses work with the team to expedite the individual's recovery and maximize healthy living.

♦ PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH THERAPY MANAGMENT - Diamond's team of therapists are experienced in treating a variety of diagnoses and have expertise in specific areas of treatment (Cognitive Rehab, NDT, etc). Our therapists participate in ongoing training and in-services in order to maintain a strong knowledge base that promotes comprehensive goal directed care. The therapy team coordinates an individualized treatment plan in order to assist the individual in reaching their maximum potential with mobility and daily living activities.

♦ HOME HEALTH AIDES- Our trained caregivers are available to assist with an individual's personal care. The aides implement a plan of care, under the supervision of a nurse that supports recovery and independence for the individual with his or her family. Services include personal care, homemaking and transportation.

♦ Additional resources include Dieticians, Social Workers, and Psychiatric Nurses to complete the professional team.

COVERAGE

Diamond Home Care works directly with case managers, adjusters, and insurance agencies to establish a cost effective plan of care to optimize rehabilitation.

STAFFING

Diamond is able to staff 7 days per week up to 24 hours a day for individualized personal assistance as well as provide additional skilled services recommended by the plan of care.
