Susan is a Registered Nurse and Certified Case Manager with 20 years experience in the home health care field. Areas of expertise include: team building, training, development and implementation of a care management model to enhance the quality services being provided to clients, survey readiness, and expanding the already excellent team of professionals and service area. Susan thoroughly believes in enjoying work and celebrating accomplishments of Team efforts. We welcome Susan and believe she is going to make a great asset to Diamond Group Health Services.

We are currently expanding our service area and awaiting Medicare survey in Grand Rapids. We are now accepting patients through our outreach programs including treatment of the noninsured or underinsured clients.

We are pleased to announce that Diamond Home Care has become CHAP certified as of November, 2006.
